Epic can put their app on the AppStore anytime they want, its not Apple, its Epic. Epic refuses to put their app on the AppStore. The EU ruling allows Epic to have their own “appstore”. Apple is forced by their government to do this. All Epic has to do is bring it back to the AppStore like it was back in the day but big companies are gonna be just that, big companies.
Altstore works in the us, here’s how to install it on MacOS, and it does work on iOS it was designed for iOS. It was made for iOS.

That is unfortunately not true. Go read the descriptions of the GeForce Now rigs. They’re all pc rigs, which means that Fortnite is running on a PC, streamed to your device. I don’t know the exact rigs as I don’t use GeForce Now myself, but I know you can play on an RTX 3060 rig for free, and a 4090 for some amount. NVidia don’t make mobile GPU’s.
Apple blocked Epic from publishing first-party apps on the App Store. Epic was allowing people to buy IAPs without going through Apple’s store to get the cut, which is against Apple’s rules on it — Apple got mad and banned them from the store.
Epic is sidestepping this by doing their own store. Apple doesn’t want this, but the EU government mandates that they have to, in an effort to de-monopolize the App Store on iOS, stating it as unlawful. That’s why it’s only available in the EU for now. It’s Apple, not Epic. Apple’s being lawfully mandated to comply.
